Accident investigation training

This course is intended for people with the responsibility for investigating accidents and incidents in the workplace.

 

This course is intended for people with the responsibility for investigating accidents and incidents in the workplace.

All employees who are going to be part of the accident investigation team as well as supervisors who will be part of any investigation of an accident should be trained. Training is recommended for all employees so they know what to expect if they are injured or witness an accident. They will know that the purpose of the investigation is to prevent another accident and not to blame someone.

The Accident Investigation course will also cover how to reduce the risk of expensive litigation and criminal proceedings and establish the importance of correct methods of investigation.

At the conclusion of the Accident Investigation course delegates will have a good understanding of the basic accident investigation process. Delegates will be able to take the initiative in finding out what went wrong, why, and how to make sure it does not happen again.

Objectives

  • Understand the key steps in an accident investigation.
  • Identify the critical information that is required to complete your company Accident Report and the Corrective & Preventative Action report.
  • Plan and conduct an accident investigation.
  • Analyse the data gathered during the investigations.
  • Apply incident investigation tools:
    • Incident Cause Analysis Method (ICAM)
    • Energy flow and barriers
    • Events and conditions charting
    • Failure mode tree
    • Human error analysis
    • Change analysis
  • Develop and evaluate corrective measures.
  • Prepare an Accident Investigation Report and a Corrective & Preventative Action Report.

Training Frequency

It is recommended that employees be trained annually so their accident investigation knowledge remains fresh.

Training records should be kept showing when the training was held, what was covered, who gave the training and who attended.
